The paper presents a case report of a 20-year-old patient treated for a year of persistent pain in the area of tooth 23. Initially, the clinical picture in the intraoral examination resembled Stillman's cleft. The medical history included extraction of tooth 24 for orthodontic reasons a year earlier and periodic secretion of bone fragments from the wound. Further CBCT imaging showed a vertical bone defect on the distal wall of tooth 23. A gingival regeneration procedure was performed using a free subepithelial connective tissue graft.
